I just installed the ISAPI rewrite V3 with the msi installer and am running IIS6 on 2003 server. Everything seems fine as far as the install goes but I cannot add a .htaccess file and if I add a test redirect in the httpd.conf file it does not appear to work. I tried putting an .htaccess in the docroot but I get an error saying I need a file extension. I went in and added a .htaccess file type to the MIME list with type text/plain. It still does not work.

Any thoughts on how to get this working?

Have you tried to use ISAPI_Rewerite Manager to add .htaccess files? Maybe you have installed Lite version that does not support .htaccess files?
As for redirects, probably the rule syntax is wrong. Try to follow troubleshooting section from documentation first.
