When do changes to .htaccess (and httpd.conf) should take effect?

Prob: Rewrite rules work well in generell, but changes to .htaccess have no effect.

Restarting the Website (in IIS-Managment console) does not help.
IIS_WPG permissions are set properly
Even renaming or deleting .htaccess has no effect - the old rewrite rules are still processed (and logged).
It needs Restart of WWW-Service to have the changed taking effect.

Questions:
1. When SHOULD changes take effect normally?
2. What could cause other behaviour?

Hi Andy.
Please make sure that ISAPI_Rewrite was not installed twice.
Also, please provide your rewrite.log and error.log.
