but my means:

 The Folder /pathology/  is a website(www.binglixue.com).

when input www.binglixue.com/cai , error.

but input www.binglixue.com/cai/ , right.

Please try to fix the rule like this:

[ISAPI_Rewrite]
RewriteCond Host: www\.binglixue\.com
RewriteRule /([^/]+)/? /pathology/$1/
