Please try to test the hardcoded rules, like this:

RewriteRule /blog/index.php/category/case-studies/ /blog/category/case-studies/ [I,R]
RewriteRule /blog/category/case-studies/ /blog/index.php/category/case-studies/ [I,L]

Use the same testing request as above: http://www.efacility.in/blog/index.php/category/case-studies/
And report the results.

I placed the rules at the top of httpd.ini file, still the same error occurs.

That's really strange...
Please make sure you don't have ISAPI_Rewrite dll registered twice - on server and site level (it should be on server level only).

If there's no progress, we'd like to get some remote access to your server. Remote Desktop would be great.
