I have recently purchased and installed Hotlink Blocker as I was having trouble with other sites linking to hi-res images. It has worked very well at dealing with that so all is good :)

However I wondered if it's possible to use any of the isapi_rewrite functionaility with Hotlink Blocker? I.e. use some of the rules that you would put in httpd.ini? Or do I have to install isapi_rewrite too for that?

Yes, you need install isapi_rewrite for this.
