Recently we have moved servers and now we have something like actual app on different server but the rewrite rule is on another server. And actual request always goes to the server where rewrite rule is and then this request to redirected to the server where the app resides. But sometime client see a blank page.

No idea why its happening. Any help will be appreciated.

There are a lot of possible reasons: wrong redirect location, intermidiate proxy server failure, application failure, client error, wrong rule, etc
To identify the problem root provide the following, please:
1) Test URL.
2) Your ruleset.
3) IIS log record, corresponding to a failed request.
4) Client-server http session log. It could be produced with the help of a browser plugin like IEHeaders or HttpWatch for IE or Live Http Headers for Firefox, or with the help of the Fiddler proxy.
