Thanks for the debug file. I've installed it, installed sysinternal's monitor and Hotlinkblocker failed to initialize.

I have removed 0.45 and installed 0.38 back, which is working fine again, as you can see on the test page:

But have you generated debug log file? This was why I sent you a debug build. Please send this debug log to our support service, it may help us to identify the problem.

I did install the debug version, and rebooted the server. I then ran the Sysinternals app, but even though an error was logged and the Debug to File option was set, no debug file was created.

Nothing was showing on the app's window dialog either...

I think you simply didn't set up correct version of .dll file. Please install HotlinkBlocker.dll file I did sent to you manually as ISAPI filter for your site. Simply click on add ISAPI filter and point to that file. After that restart IIS and you should see debug log messages in DebugView tool.
