I have installed 0.45 over .38 on my server and when I look at the ISAPI list it shows HotlinkBlocker with Priority "Unknown" and I can't move it up or down - it also seems not to be working...

Any ideas?

Hmmm. Just rebooted the server (not to do it everyday) and it now says "Medium".

This is normal since IIS don't load filters immediately. After you access some pages on your site and restart IIS console you should see current filter status.
