Does the QSA flag do the same thing as adding &%{QUERY_STRING} to the output string?  If so, is one more efficient than the other?

QSA is more intellectual than &%{QUERY_STRING} because it appends query string (&param=value) if some query string already exists, and adds query string (?param=value) if there's no existing query string.
