If you have "RewriteLogLevel 9" line in httpd.conf, the log should be more detailed. Could you please provide all rewrite log records corresponding to the request.

We figured out the problem. The Network Service user didn't have sufficient permissions for the rewrite engine on the sites files.

Thanks for getting us pointed in the right direction with the debug.
