I'm currently using ISAPI Rewrite

I'd interesting in moving my ColdFusion app to Apache and would therefore
like to use MOD_Rewrite. Is this possible? Or do I need to rewrite all the
rewrites?

If you are on ISAPI_Rewrite v2, you'll need to rewrite the rules to mod_rewrite syntax.
If you use v3, the syntax will work as is.

Is there a tool available anywhere to rewrite the v2 httpd.ini rules to mod_rewrite format rules?

There's a conversion tool available in ISAPI_Rewrite3 manager.
It's a button "Import from v2".
Sometimes some minor manual fixes may be necessary.
